Qualifications

• An associate degree with 5+ years of related experience (Residential Case Manager I) will also be considered.

Responsibilities

• Work with an assigned caseload to provide support services.
• Interact with residents and serve as a role model for program participants.
• Implement individual service plans to help residents reach rehabilitation goals.
• Maintain case records, progress notes and service plans.
• Provide counseling and lead groups in recreational and rehabilitation activities.
• Escort clients to medical or benefit appointments and coordinate community activities.
• Manage crises and document events promptly in incident reports.
