Job summary

ICL in New York is seeking a field-based professional to support individuals with Serious Mental Illness, developmental disabilities, and related conditions. You will provide home visits, counseling, service coordination, and advocacy to help clients access medical, social, educational, vocational, and financial resources.

The role involves crisis intervention, documentation, collaborating with care managers and external providers, and conducting safety checks in consumer residences.

Qualifications

  • Bachelor's degree in a human services field.
  • Two years of experience providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED.
  • Must be able to travel within the community; a valid driver’s license may be preferred depending on assignment.
  • Must be authorized to work in the U.S.
  • Onsite / Community-based field work with regular travel to consumer residences.

Responsibilities

  • Conduct home visits to provide supportive counseling and independent living assistance.
  • Help consumers modify behaviors, build insight, and develop coping skills.
  • Assess needs, gather medical/psychosocial information, and develop individualized service plans.
  • Maintain accurate case records, documentation, assessments, and quarterly reviews.
  • Provide 1:1 and group counseling using cognitive‑behavioral and skills-training techniques.
  • Coordinate services with care managers, ICL staff, and external providers.
  • Assist consumers in accessing community resources, benefits, and vocational opportunities.
  • Perform crisis assessment and intervention; refer to appropriate services.
  • Accompany consumers to medical, social service, and government appointments.
  • Report incidents and complete incident documentation per policy.
