Intermountain Centers is seeking a Care Coordinator to manage members’ whole health using an integrated case management model. You will assess, plan, implement and monitor services, and document encounters in the Electronic Health Record.

The role requires collaboration with the member’s support system and care team, advocacy for resources, and strong documentation and de-escalation skills to support effective service plans.

Qualificações

• Bachelor’s degree in behavioral health or social work or equivalent.
• Associate’s degree with 2 years related experience.
• High school diploma with 4+ years related experience.

Responsabilidades

• Observe members and document concerns in clinical records.
• Monitor health and safety and report concerns per policy.
• Coordinate services and act as an advocate for resources.
• Read and record assessments, measures and criteria accurately.
• Maintain member records and treatment plans with progress notes.
• Collaborate with primary care and interdisciplinary teams.
• Develop time-specific, action-oriented service plans.
• Complete required trainings and absorb policy updates.
• May transport members in company vehicles when needed.
