Job summary

Health Connect America, Inc. seeks an Integrated Care Extender to support care coordination across homes, communities, and offices. You will work with Integrated Care Coordinators to ensure timely outreach, appointments, and resource referrals.

Qualifications

• TN requires a Bachelor's degree; human services preferred.

Responsibilities

• Support Integrated Care Coordinators with care coordination tasks.
• Perform outreach and follow-up with members.
• Coordinate services and appointments including reminders and transportation.
• Engage in health promotion activities and knowledge sharing.
• Share member information with care team to address needs.
• Provide and track referrals to community resources.
• Participate in case conferences and team meetings.
• Assist in assessing unmet health-related resource needs.
• Document efforts in ONEcare timely.
