Quality Healthcare Development seeks a Nurse Care Coordinator to design and implement a comprehensive care coordination program in partnership with the Quality Director. You will engage patients within the ACO population to improve outcomes and reduce costs, coordinating care across the continuum with families and providers.

The role emphasizes patient engagement, self-management, and health literacy, acting as a liaison to community resources and ensuring timely follow-up and appropriate care

Formación

• BSN required.
• Minimum five years in a clinical or outpatient setting preferred.
• Current RN license in the State of South Carolina required.
• Experience in care coordination, population health, or value-based care preferred.

Responsabilidades

• Participate in the development, evaluation, and improvement of the Care Coordination Program.
• Coordinate patient outreach, follow-ups, and utilization of services.
• Engage patients and families to promote self-management and shared decision-making.
• Develop care plans with providers to maximize outcomes and cost-effective care.
• Assist patients in accessing community resources and financial assistance.
