CareSource in the United States seeks a Manager, Case Management - Integrated Care to lead the Integrated Care Duals Product Teams in coordinating care, services and benefits for selected member populations.

The role oversees the performance of the Integrated Care Management Liaisons and CM teams, ensures compliance with state and accreditation requirements, and travel to member and provider sites as needed. A RN license and case management experience are required.

Qualifications

• Bachelor’s Degree in Nursing or related field required.
• Minimum of five years clinical experience required.
• Three years Medicaid/Medicare or HCSB Waiver case management experience preferred.
• Two years of management experience preferred.

Responsabilités

• Oversee Integrated Care Duals Product Teams to coordinate care and benefits for members.
• Monitor utilization and manage clinical variance and benefits.
• Lead and evaluate Integrated Care Management teams to ensure quality outcomes and compliance.
• Oversee care planning and transitions between care settings with community partners.
• Ensure staffing, scheduling, training, and performance improvement across teams.
• Travel to member and provider sites as needed to support program delivery.
