Job summary

CareSource seeks a Manager, Case Management - Integrated Care to oversee the Integrated Care Duals product teams, coordinating care, services and benefits for selected member populations.

The role emphasizes leadership of care management staff, ensuring safe, compliant and high-quality outcomes, with ongoing review of care plans and collaboration with stakeholders. Travel to sites and providers is required.

Qualifications

• Bachelor’s Degree in Nursing, healthcare field, business admin or related field required
• Minimum of five years of clinical experience
• Three years Medicaid/Medicare managed care or HCSB Waiver case management preferred
• Two years of travel/field work may be required
• Excellent communication and leadership abilities

Responsibilities

• Oversee integrated care product teams coordinating care and benefits
• Monitors safe and effective utilization of resources and compliance
• Manage performance of care management staff and ensure quality outcomes
• Review care plans, interventions and ensure appropriate transitions
• Travel to member and provider visits as needed to support care
