Job summary

CareSource is seeking a Manager, Case Management - Integrated Care to oversee the Integrated Care Duals Product Teams and coordinate care, services, and health benefits for selected member populations. You will lead the care management teams to ensure safe, effective service delivery and quality outcomes across states and partners.

You will monitor utilization, manage team performance, and ensure adherence to state and accreditation requirements while guiding staff development and program

Qualifications

• Bachelor’s Degree in Nursing or related field required.
• Minimum 5 years clinical experience required.
• Minimum 3 years Medicaid/Medicare managed care experience preferred.
• 3 years as a case manager preferred.
• 2 years management experience preferred.

Responsibilities

• Oversee Integrated Care Duals Product Teams and coordinate care, services and health benefits.
• Monitor utilization and manage resources for safe, effective care.
• Lead care management teams to ensure quality outcomes and compliance.
• Ensure care plans are implemented, evaluated, and updated.
• Travel to member and provider sites as needed to support program operations.
