Job summary

CareSource is seeking a Manager, Case Management - Integrated Care to oversee the Integrated Care Duals Product Teams, coordinating care, services, and health benefits for selected member populations.

The role focuses on leading care teams, ensuring safe, effective service delivery, and compliance with state and accreditation standards, while maintaining optimal member outcomes across travel and multiple sites.

Qualifications

• Bachelor’s Degree in Nursing or related field required
• Minimum 5 years clinical experience required
• Medicaid/Medicare managed care or HCSB Waiver case management experience preferred
• Three years as a case manager preferred
• Two years management experience preferred

Responsibilities

• Oversee Integrated Care Management teams to ensure safe service delivery and quality outcomes
• Monitor utilization and benefits management for optimal care
• Verify eligibility and health status of members as appropriate
• Audit documentation for thoroughness and accuracy in care management
• Coordinate services with community partners to maximize appropriate resources
• Travel to member and provider visits as needed to ensure program effectiveness
