Job summary

CareSource seeks a Manager, Case Management - Integrated Care to lead the Integrated Care Duals Product Teams, coordinating care, services and health benefits for selected member populations. You will oversee day-to-day operations, ensure regulatory compliance, and drive performance across care management teams.

The role requires RN or related licensure, minimum five years clinical experience, Medicaid/Medicare managed care experience preferred, and strong leadership and communication skills

Qualifications

• Bachelor’s Degree in Nursing, healthcare field, business administration or related field is required.
• Minimum of five (5) years or more of clinical experience is required.
• Minimum of three (3) years Medicaid and/or Medicare managed care or HCSB Waiver case management experience is preferred.
• Three (3) years as a case manager is preferred.
• Two (2) years of management experience is preferred.

Responsibilities

• Oversee Integrated Care Duals Product Teams to coordinate care, services, and health benefits for specified member populations.
• Monitor utilization, ensure safe and effective service delivery, and maintain compliance with state and accreditation requirements.
• Manage performance of Integrated Care Management teams, conduct staff meetings and trainings, and ensure documentation is complete and accurate.
