Qualifications

• Bachelor’s Degree in Nursing, healthcare field, business administration or related field or equivalent years of relevant work experience required.
• Minimum of five (5) years or more of clinical experience is required.
• Case Management certification is preferred and Registered Nurse (RN) license in Ohio is required or LPCC/LISW/psychologist as applicable.

Responsibilities

• Oversees Integrated Care Duals Product Teams and coordinates care, services and benefits.
• Manages performance of Care Management teams to ensure safe, effective service delivery and compliance.
• Ensures eligible member status, enrollment history, and health status are accurately verified.
• Maintains care plans, measures outcomes, and monitors plan effectiveness.
• Oversees scheduling, staffing, training and quality/performance improvement.
