Job summary

You will provide consultation, co-management of mental health issues, and coordinate care with an interdisciplinary team across clinic sites, aiming for high-quality, patient-centered outcomes.

Qualifications

• Master’s degree in the Behavioral Health field with 2 years of related experience.
• Certification or eligibility as LCSW Ed.D, Psy.D., LPC or Ph.D. is required.
• Computer literacy is required; bilingual English/Spanish preferred; knowledge of psycho-pharmacology.

Responsibilities

• Provide behavioral health services to patients within a primary care setting.
• Provide clinical consultation to providers and staff for mental and behavioral health issues.
• Conduct assessments and DSM/ICD diagnoses for referred patients.
• Deliver brief, evidence-based behavioral interventions and psycho-education.
• Collaborate with multidisciplinary teams and co-manage patient care.
• Attend staff meetings and participate in care coordination and crisis triage as needed.
