Job summary

Parsons Family Health Center in Phoenix, AZ is seeking a Behavioral Health Consultant to support primary care teams. You will assess psychosocial factors, deliver brief interventions, and coordinate care to improve health outcomes for patients facing mental health and housing challenges.

The role requires a Master’s degree in Counseling or Social Work, relevant licensure, and experience in health care. BLS certification and a driver license are required; bilingual skills preferred.

Qualifications

• Master’s degree in Counseling, Social Work or Doctorate in Psychology required.
• Must possess Arizona BBHE license (LPC, LCSW or Psychologist) or equivalent.
• Two years of experience in the health care field.
• Current Arizona Driver License and Basic Life Support (BLS) certification required.
• Bilingual in English and Spanish preferred.

Responsibilities

• Provides services within the integrated Primary Care Behavioral Health model.
• Completes comprehensive psychosocial assessments addressing mental health and substance use.
• Develops patient-centered treatment plans with measurable goals.
• Monitors progress and adjusts plans as needed.
• Provides brief interventions and short-term counseling for depression, anxiety, trauma, and related issues.
• Connects patients to community services and resources for housing and homelessness.
