Job summary

Circle the City seeks a Behavioral Health Consultant in Phoenix, AZ to support integrated Primary Care Behavioral Health services. You will perform psychosocial assessments, provide brief interventions, and work within a multidisciplinary team to improve patient health outcomes.

Responsibilities include connecting patients to community services addressing homelessness, maintaining secure documentation in the EHR, and coordinating care with providers while adhering to confidentiality standards.

Qualifications

• Master's degree in Counseling, Social Work or Doctorate in Psychology required.
• Ability to work flexibly with a patient care team in various settings.
• Current Arizona licensure (LPC/LCSW/Psychologist) or eligibility should be noted.
• Two years of healthcare field experience.
• BLS certification.
• Arizona driver license.

Responsibilities

• Provides services within the integrated Primary Care Behavioral Health model.
• Completes comprehensive psychosocial assessments addressing mental health, substance use, homelessness-related issues.
• Coordinates with the multidisciplinary team to develop patient-centered treatment plans.
• Delivers brief interventions and short-term counseling for depression, anxiety, trauma, and related concerns.
• Maintains confidentiality and documents within the electronic health record.
• Connects patients to community services and resources addressing housing and shelter needs.
