Job summary

Adelante Healthcare Mesa seeks a Behavioral Health Therapist to provide comprehensive therapy for individuals and families, using CBT, ACT, mindfulness, and related approaches. The role includes completing intakes, developing and adjusting treatment plans, and coordinating care within a multidisciplinary team.

Qualifications

• Provide behavioral health services to children, adults, and families.
• Completes initial intake and diagnostic assessments with measurable objectives.
• Develops initial treatment plans and updates plans as needed.
• Provides quality clinical consultation to providers and staff.
• Maintains familiarity with regulatory requirements and chart documentation.

Responsibilities

• Performs initial assessments and DSM-5/ICD-10 diagnoses.
• Delivers evidence-based psychotherapy in individual and group settings.
• Coordinates care with psychiatrists, PCPs and care managers.
• Maintains patient registry and follows up on missed appointments.
• Documents clinical interventions and patient referrals.
