Job summary

Adelante Healthcare Mesa, AZ is seeking a Behavioral Health Case Manager to manage a caseload, assess client needs, and develop service plans with coordinated care across internal and external resources.

The role requires a HS diploma (Associate degree preferred), strong customer service and computer skills, good knowledge of DSM/ICD, CPR/AED training, and the ability to work with diverse populations; Bilingual English/Spanish is strongly preferred.

Qualifications

• High school diploma or GED required; Associate’s Degree preferred.
• Minimum of one year of experience in behavioral health preferred.
• Excellent customer service skills and computer literacy.
• Thorough knowledge of DSM/ICD; able to complete assessments with oversight.
• CPR/AED certification; valid Arizona driver’s license and fingerprint clearance.
• Bilingual English/Spanish strongly preferred.

Responsibilities

• Screen new service requests and ensure clients are served per policy.
• Participate in regular interdisciplinary staff meetings.
• Accept cases and complete case management assessments.
• Develop treatment/service plans with clients and coordinate care.
• Provide direct client services and maintain case records timely.
• Communicate with psychiatrist and health team about medications.
