Job summary

Adelante Healthcare is hiring a Behavioral Health Case Manager in Phoenix. You will manage an assigned caseload, assess client needs, and develop service plans with coordination across internal and external resources.

Qualifications

• High school diploma or GED required; Associate’s degree preferred.
• Minimum of one year of experience in Behavioral Health preferred.
• Excellent customer service and computer skills.
• Thorough knowledge of DSM and ICD preferred.
• Ability to complete a Behavioral Health Assessment with clinical oversight.
• Bilingual English/Spanish strongly preferred.
• Valid Arizona driver’s license.
• Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety.

Responsibilities

• Screen and assess new service requests within the center according to policies.
• Participate in regular interdisciplinary staff meetings.
• Accept new cases after intake; complete case management assessments.
• Develop treatment/service plans with clients and build effective alliances.
• Provide direct client services including case management and counseling with clinical oversight.
• Conduct face-to-face reviews; adjust service plans to meet goals.
• Document all service contacts timely and maintain case records.
• Coordinate with psychiatrist and Behavioral Health team regarding medications.
