Job summary

Parsons Family Health Center in Phoenix, AZ is seeking a Behavioral Health Consultant to join the primary care team. The role focuses on identifying psychosocial factors affecting health, providing brief interventions, and coordinating care within a multidisciplinary group.

The position requires a Master’s degree in Counseling, Social Work or a Doctorate in Psychology and current Arizona licensure. Strong collaboration, documentation, and bilingual English/Spanish abilities are preferred.

Qualifications

• Must be (at least) 21 years old.
• Experience with psychosocial assessment and treatment planning.
• Proficient in counseling theories and interventions.
• Ability to collaborate with professionals from various services.
• Maintain up-to-date documentation of clinic encounters in an EHR.
• Confidentiality in accordance with policies and regulations.

Responsibilities

• Provides services within the integrated Primary Care Behavioral Health model.
• Completes comprehensive assessments identifying psychosocial factors affecting health management.
• Uses screening tools (ASAM, DAST, AUDIT, PHQ-9, GAD-7) as needed.
• Develops patient-centered treatment plans with measurable goals.
• Monitors progress toward identified goals and objectives.
• Delivers brief/short-term interventions (motivational interviewing, coping skills, etc.).
• Works with patients in crisis for rapid intervention, risk assessment and safety planning.
• Connects patients to community resources addressing homelessness-related needs.
• Maintains confidentiality and completes required documentation.
