Job summary

Mindoula Health is expanding its Collaborative Care Program in Oregon. The Behavioral Health Integration Clinician collaborates with medical providers and psychiatric consultants to deliver high-quality behavioral health care within primary care settings.

You will assess patients, develop treatment plans, provide psychotherapy, coordinate care, and monitor outcomes using the EHR and clinical workflows in a hybrid in-clinic and telehealth environment.

Qualifications

• Master's degree in relevant field (Social Work, Psychology, Counseling or Rehabilitation).
• 3-5+ years of behavioral health intervention, case management or care coordination experience.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, Psychologist).
• Medicaid/Medicare enrollment preferred but eligibility required.
• Hybrid work model with telephonic/video services; high speed internet required.
• Proficient with EHR, scheduling and remote communication tools.

Responsibilities

• Screen and assess patients for mental health and substance use disorders.
• Develop treatment plans and provide psychotherapy.
• Coordinate care across medical and psychiatric providers.
• Track treatment response and document in EHR.
• Facilitate referrals for evidence-based psychosocial treatments.
• Participate in weekly caseload consultation with psychiatric consultant.
• Meet performance metrics set by clinical leadership.
