Job summary

Mindoula Health's Collaborative Care Program (CCP) in Oregon seeks a Behavioral Health Integration Clinician to join the collaborative care team. You will perform assessments, develop treatment plans, provide psychotherapy, and coordinate mental and physical healthcare for an assigned patient caseload.

Responsibilities include risk assessment, evidence-based interventions, medication support, and regular consultation with psychiatric consultants.

Qualifications

• Master's degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field.

Responsibilities

• Screen and assess patients for mental health and substance use disorders; engage in treatment planning and follow-up.
• Provide brief behavioral interventions using evidence-based techniques.
• Support psychotropic medication management with focus on adherence and side effects.
• Coordinate care across mental and physical health services through EHR documentation.
• Track treatment response and outcomes via in-person, video, or telephonic encounters.
• Educate patients and facilitate referrals to evidence-based psychosocial treatments.
