Job summary

Mindoula Health’s Collaborative Care Program (CCP) is expanding in Oregon, integrating behavioral health into primary care to improve quality of life for patients with behavioral health challenges and chronic medical conditions.

The Behavioral Health Integration Clinician is a central member of the care team, providing assessments, treatment planning, psychotherapy, and coordination of mental and physical health care for an assigned caseload.

Qualifications

• Master's degree in Social Work, Psychology, Counseling, Rehabilitation, or related field.
• Experience in behavioral health intervention, case management, and/or community care coordination.
• Experience with older adult Medicare populations with chronic medical and behavioral health challenges.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, or Psychologist).
• Current enrollment in Medicaid/Medicare preferred but eligibility required.
• Hybrid work with telephonic/video services; high-speed home internet required.
• Proficient with EHR, scheduling systems, remote phone systems and video systems.

Responsibilities

• Screen and assess patients for mental health and substance use disorders; plan follow-up care.
• Provide brief behavioral interventions using evidence-based techniques.
• Support psychotropic medication management as prescribed by medical providers.
• Coordinate health behavior and social determinants of health interventions.
• Track treatment response and monitor patients via in-person, video or telephone.
• Educate patients about mental health and treatment options.
• Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments.
• Participate in weekly caseload consultations with the psychiatric consultant.
• Document progress and treatment recommendations in EHR and share with teams.
• Meet performance metrics each month and contribute to process improvements.
