Qualifications

• Master’s degree in Social Work, Psychology, Counseling, Rehab or related field.
• 3–5+ years in behavioral health intervention, case management or community care coordination.
• Experience with diverse populations including older adult Medicare.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, Psychologist).
• Medicaid/Medicare enrollment preferred but eligibility required.
• Hybrid work setup with telehealth and occasional in-person visits; reliable home internet.
• Proficiency with EHR, scheduling systems, and remote/ video systems.

Responsibilities

• Screen and assess patients for mental health and substance use disorders; develop treatment plans.
• Provide brief behavioral interventions using evidence-based techniques.
• Support psychotropic medication management with adherence monitoring.
• Coordinate health and social determinants of health for patients.
• Track treatment response and document progress in EHR.
• Facilitate referrals to evidence-based psychosocial treatments as needed.
• Attend weekly caseload consultations with the psychiatric team.
• Meet monthly performance metrics set by clinical leadership.
