Job summary

Mindoula Health is expanding its Collaborative Care Program in Oregon. The Behavioral Health Integration Clinician will work within a core team to assess patients, develop treatment plans, and provide psychotherapy, coordinating both behavioral and medical care.

The role includes monitoring treatment response, documenting in the EHR, and collaborating with the psychiatric consultant. A master’s degree and an active Oregon license are required; the position is hybrid with telehealth and in-clinic

Qualifications

• A master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or a related field.
• 3–5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, or Psychologist).
• Current enrollment in the state’s Medicaid & Medicare is preferred but eligibility is required.

Responsibilities

• Screen and assess patients for mental health and substance use disorders; facilitate engagement, treatment plans and follow-up.
• Provide psychotherapy and develop treatment plans; coordinate care with medical and psychiatric teams.
• Support psychotropic medication management and monitor adherence, side effects, and effectiveness.
• Utilize EHR and clinical workflows to document encounters and track outcomes.
• Participate in weekly caseload consultations with the psychiatric consultant.
