Job summary

Mindoula Health's Collaborative Care Program (CCP) is expanding to Oregon, embedding behavioral health into primary care to improve patient outcomes. The Behavioral Health Integration Clinician leads assessment, treatment planning, and coordination for an assigned caseload, with weekly consultations with a psychiatric consultant.

Qualifications

• Master’s degree in social work, psychology, counseling, rehabilitation, or other relevant field.
• 3-5+ years of experience in behavioral health intervention clinical work, case management, and/or community care coordination.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, Psychologist) or license eligible.
• Remote work with state-required presence; stable high-speed internet.
• Experience with electronic medical records and care coordination workflows.

Responsibilities

• Screen and assess patients for mental health and substance use disorders; engage in treatment planning and follow-up.
• Provide brief behavioral interventions using evidence-based approaches and clinical software.
• Support psychotropic medication management with adherence monitoring and side-effect tracking.
• Coordinate care across medical, psychiatric, and community providers.
• Document progress in EHR and communicate recommendations to care team.
• Attend weekly caseload consultations with the psychiatric consultant.
