Job summary

Mindoula Health Inc. is expanding its Collaborative Care Program in Oregon.

The Behavioral Health Integration Clinician will provide assessment, develop treatment plans, and deliver evidence-based psychotherapy to an assigned caseload, coordinating care with medical providers and psychiatric consultants. You will track outcomes in the EHR, facilitate in-clinic or telehealth sessions, and participate in weekly consultation meetings to optimize collaborative workflows.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or related field.
• 3–5+ years of behavioral health intervention clinical work, case management, and/or community care coordination.
• Experience with diverse populations, including older adult Medicare populations with chronic conditions.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, or Psychologist).
• Current enrollment in Oregon Medicaid/Medicare preferred or eligible.
• Hybrid work with telephonic/video services; high‑speed internet required.
• Proficiency with electronic health records (EHR) and scheduling/remote systems.

Responsibilities

• Screen and assess patients for mental health and substance use disorders and coordinate follow-up care.
• Provide psychotherapy and develop treatment plans for assigned caseload.
• Coordinate community and medical/behavioral health care as part of the care team.
• Monitor treatment response and document progress in the EHR.
• Facilitate referrals to evidence-based psychosocial treatments as clinically indicated.
• Participate in weekly caseload consultation with psychiatric consultant and communicate recommendations.
• Meet performance metrics set by clinical leadership.
