Job summary

Mindoula Health's Collaborative Care Program in Oregon seeks a Behavioral Health Integration Clinician to join the patient-centered care team. Qualifications

• Master’s degree in relevant field (Social Work, Psychology, Counseling, Rehabilitation).
• 3–5+ years in Behavioral Health interventions or care coordination.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, Psychologist).

Responsibilities

• Screen and assess patients for mental health and substance use disorders.
• Provide brief behavioral interventions using evidence-based techniques.
• Support psychotropic medication management and monitor adherence.
• Coordinate care with medical and psychiatric providers and track outcomes in EHR.
• Facilitate referrals to psychosocial treatments and document progress.
• Participate in weekly caseload consultation and align with clinical directions.
