Job summary

Care Partners At Home is seeking an Intake Specialist to serve as the primary contact for new clients and families. You will assess needs, coordinate with Regional Centers and Cal AIM, and help design personalized care plans with our care teams.

You will maintain CRM records, ensure regulatory compliance, and collaborate with management to improve intake processes while fostering compassionate, professional service. Growth and team support are emphasized.

Qualifications

• Experience in intake coordination, case management, or related roles.
• Excellent interpersonal and communication skills with compassion.
• Proficiency with computer systems including Office, CRM, and web apps.
• Ability to pass a federal background check and have a valid driver's license.

Intake Specialist Requirements:

EDUCATION AND EXPERIENCE

• Experience in intake coordination, case management, or a related role.
• Exceptional interpersonal and communication skills, with a compassionate and professional demeanor.
• Proficiency in computer systems, including Microsoft Office, CRM platforms, and internet-based applications.
• Ability to pass a federal background screen.
• Valid driver’s license and proof of current auto insurance.

Essential Functions:

• Client Intake and Coordination:Serve as the primary point of contact for new clients and their families.Conduct thorough intake assessments to identify client needs and preferences. Collaborate with Regional Centers, Cal AIM, caregivers, and internal teams to design and implement personalized care plans.
• Documentation and Compliance:Maintain accurate records and documentation in our CRM system.Ensure all intake processes comply with state regulations and Regional Center and or Cal AIM requirements.
• Relationship Building:Develop and maintain positive relationships with clients, families, and Regional Center and or Cal AIM representatives.Act as a trusted resource and advocate for families navigating theservices system.
• Team Collaboration:Work closely with the Management and Operations teams to streamline processes and ensure high-quality service delivery.Provide feedback and recommendations to enhance the client intake experience.
