Job summary

Care Partners At Home in Irvine, CA is seeking an Intake Specialist to manage new client intake, assess needs, and coordinate with caregivers and regional partners. The role is full-time and on-site with a comprehensive benefits package.

You will document in our CRM, maintain compliant records, and build strong relationships with families, regional centers, Cal AIM, and internal teams to design personalized care plans. A compassionate, detail-oriented mindset is essential for success.

Qualifications

• Experience in intake coordination, case management, or a related role.
• Exceptional interpersonal and communication skills, with a compassionate and professional demeanor.
• Proficiency in computer systems, including Microsoft Office, CRM platforms, and internet-based applications.
• Ability to pass a federal background screen.
• Valid driver’s license and proof of current auto insurance.

Responsibilities

• Client intake and coordination: primary contact for new clients and families; design personalized care plans with internal teams.
• Documentation and compliance: maintain accurate CRM records and ensure regulatory compliance.
• Relationship building: develop and maintain relationships with clients, families, and regional centers or Cal AIM representatives.
• Team collaboration: work with management to streamline processes and improve intake experience.

Job description

• Compensation:$24 - $29/hr (Negotiable, based on experience)
• Type: Full-Time,ON-SITE
• Schedule: Monday - Friday; 8:00 am-5 pm

Intake Specialist Requirements:

EDUCATION AND EXPERIENCE

• Experience in intake coordination, case management, or a related role.

Essential Functions:

• Client Intake and Coordination:Serve as the primary point of contact for new clients and their families.Conduct thorough intake assessments to identify client needs and preferences. Collaborate with Regional Centers, Cal AIM, caregivers, and internal teams to design and implement personalized care plans.
• Documentation and Compliance:Maintain accurate records and documentation in our CRM system.Ensure all intake processes comply with state regulations and Regional Center and or Cal AIM requirements.
• Relationship Building:Develop and maintain positive relationships with clients, families, and Regional Center and or Cal AIM representatives.Act as a trusted resource and advocate for families navigating theservices system.
• Team Collaboration:Work closely with the Management and Operations teams to streamline processes and ensure high-quality service delivery.Provide feedback and recommendations to enhance the client intake experience.
