Muir Home Health in the East Bay, CA is seeking an experienced Scheduling/Intake Coordinator to join our team. This 100% in-office position offers strong compensation and comprehensive benefits.

You will manage patient intake, data entry, verify insurance, obtain authorizations, and run daily stand-up meetings. You will coordinate clinician scheduling in Home Care Home Base and align staff with patient needs.

Responsabilidades

• Direct and manage all daily patient referral, intake, and scheduling operations.
• Ensure compliance with state, federal, Medicare, and Joint Commission requirements related to intake, scheduling, and service delivery.
• Coordinate seamless patient transitions to home care by overseeing patient intake, education, plan of care initiation, and collaboration with multiple service providers.
• Verify insurance coverage, obtain authorizations, and support maximum third-party reimbursement, including negotiating service pricing within established guidelines.
• Maintain accurate data on staff demographics, skill sets, availability, and productivity to align patient needs with resources.
• Serve as the primary point of contact for inbound calls from patients, families, staff, and referral sources.
• Establish and maintain relationships with referral sources, patients, families, clinical staff, contract staff, and internal teams.
• Facilitate daily intake or staffing stand-up meetings and support leadership with workflow prioritization.
• Accurately complete and manage scheduling-related workflows, including new referrals, missed visits, and staffing changes.
• Assist staffing with new patient referrals and identifying needs for additional staff resources.
