Job summary

Muir Home Health in the East Bay is seeking an experienced Scheduling/Intake Coordinator to manage patient intake, data entry, insurance verifications, and clinician scheduling. This 100% in-office role focuses on coordinating with patients, families, referral sources and staff to ensure seamless care delivery.

You will use Home Care Home Base, oversee daily stand-up meetings, maintain accurate records, and help optimize workflow while building strong relationships with providers and payers.

Qualifications

• Experience in intake and scheduling for home health or healthcare services.
• Proficiency with data entry and insurance verification processes.
• Ability to coordinate staffing and prioritize tasks in a clinical setting.

Responsibilities

• Direct and manage daily patient referral, intake, and scheduling operations.
• Ensure compliance with regulatory requirements related to intake and service delivery.
• Coordinate patient transitions to home care and education/plan of care initiation.
• Verify insurance coverage and obtain authorizations to support reimbursement.
• Manage clinician scheduling and staffing to align patient needs with resources.
• Serve as primary contact for inquiries from patients, families and staff.
• Maintain relationships with referral sources and internal teams to support care delivery.
• Facilitate daily intake or staffing stand-up meetings and prioritize tasks.
