Job summary

The Central Intake Coordinator at Siloam Mission serves as the primary contact for referrals to housing, MTCU, and Case Worker programs. The role coordinates referrals, conducts initial eligibility screening, and supports accurate recordkeeping and interdepartmental collaboration.

Qualifications

• Excellent verbal and written communication and interpersonal skills.
• Knowledge of homelessness, addiction, and mental health issues is an asset.
• Understanding of person-centred, harm-reduction, trauma-informed, and culturally responsive practices.
• Strong technical skills including Microsoft Office and ability to learn new software.
• Ability to type 40 WPM.
• Adaptable, dependable, and accountable.

Responsibilities

• Serve as the primary point of contact for referrals to housing, MTCU, and Case Worker services.
• Complete preliminary eligibility screening and direct referrals for secondary assessment.
• Communicate eligibility outcomes and status to referral sources and coordinate handoffs.
• Maintain centralized referral tracking and caseload information.
• Prepare monthly intake referral reports for management.
• Maintain accurate housing, referral, and program records.
