Job summary

TN Voices is seeking an Intake Coordinator to serve as the primary contact for referrals and family access to Rural THRIVE Tennessee services. The role involves gathering intake information, coordinating with Family THRIVE Coaches, and ensuring timely connections to program staff.

The position supports a streamlined, family-centered intake experience, collaborating with leadership and staff to ensure referrals are documented, assigned, and followed through.

Qualifications

• Bachelor’s degree in social work, psychology, human services, behavioral health, public health, healthcare administration, or a related field preferred.
• Minimum of two years of intake, referral coordination, customer service, healthcare, behavioral health, family services, or a related human-service setting preferred.
• Experience with electronic health records, referral-management systems, or other electronic data systems preferred.
• Knowledge of healthcare, behavioral health, social services, and community resources in Tennessee preferred.

Responsibilities

• Intake and Referral Coordination: Serve as the primary contact for referrals and service inquiries, route referrals to appropriate regional teams and coaches.
• Participant Engagement & Access: Provide trauma-informed, family-centered intake experience and identify barriers to service access.
• Case Assignment & Service Navigation: Route referrals and facilitate timely access to services based on participant needs.
• Data Management & Referral Tracking: Maintain accurate intake and referral records and monitor status.
