Job summary

Housing Works seeks an Intake Coordinator for the Emergency & Transitional Housing Program (ETH) in New York. You will manage referrals, screen eligibility, and coordinate admissions, liaising with partners and internal teams to place applicants appropriately.

The role involves data entry, waitlist management, and reporting to support service delivery and program operations. A Bachelor’s or Associate’s degree with relevant experience is required, with bilingual Spanish preferred.

Qualifications

• Bachelor’s Degree in social services, psychology, social work or related field.
• Associate’s Degree with 2+ years in social services or data entry.
• Experience in health care or nonprofit setting preferred.
• Strong written documentation and communication skills.

Responsibilities

• Review census and submit daily vacancy reports.
• Screen referrals for ETH Program eligibility and site placement.
• Gather supporting documentation to determine eligibility.
• Coordinate with interdisciplinary teams for care level assessments.
• Manage waitlist and admission communications.
• Produce regular reports for stakeholders.
• Maintain accurate data entry and EMR charts.
