Job summary

HousingPlus is seeking an Outreach Intake Coordinator for the Justice Works program, reporting to the Senior Program Director. This role involves screening referrals for eligibility, coordinating with subcontractors, and maintaining occupancy and intake data to meet contractual goals.

The coordinator will visit partners and sites, including weekly visits to Rikers Island, to ensure smooth referrals and program performance.

Qualifications

• Associate degree required; Bachelor’s degree preferred.
• 3–5 years of experience in social services/nonprofit settings, NYC-based.
• Experience with housing-first, harm-reduction and trauma-informed care helpful.
• Knowledge of professional ethics and conduct.
• Strong communication and coordination abilities.

Responsibilities

• Review and screen referrals for eligibility per MOCJ contract and subcontractor rules.
• Collaborate with the Senior Program Director and partners to match referrals to services.
• Update occupancy reports and logs, track bed availability across the consortium.
• Maintain contact with ATI, public defenders, and community agencies; attend partner meetings.
• Manage referral wait lists and participate in supervision and case reviews.
• Attend agency meetings and training; perform duties as assigned by leadership.
