Job description

Housing Works is a healing community of people living with and affected by HIV/AIDS. Our mission is to end the dual crises of homelessness and AIDS through relentless advocacy, the provision of lifesaving services, and entrepreneurial businesses that sustain our efforts.

Housing Works Health Home Care Coordination program works in partnership with medical and behavioral health providers and other community-based partners with a goal of aligning care coordination services that promote access to care and enhanced health outcomes for Medicaid recipients with unstable chronic conditions. The Health Home model of care uses comprehensive care management to address medical and behavioral health needs as well as social determinants of health like housing and benefits navigation.

Intake Coordinators are an integral part of the team who are responsible for promoting the Health Home program, screening clients for program eligibility and appropriateness, and completing program intakes, while adhering to agency and program policies and procedures around intake and enrollment. The IC is clinic and community based, responsible for working with the clinic teams and the Community Partnerships teams to complete timely eligibility assessments and intakes for referred clients. The IC is also responsible for promoting the Health Home program to clinic patients, agency programs and clients, and community partners and community members via event-based engagement. Performance is measured by meeting monthly target metrics and completing required documentation.

Responsibilities:

1. Complete screenings for all referrals received (top-down/external/internal) daily to assess eligibility and appropriateness for program enrollment.
2. Complete social care need screenings for identified clients using the identified platforms (For example, UniteUs).
3. Ensure client is properly consented for the program (DOH5055, HIPPA, Healthix, BRONX RHIO, CODE OF CONDUCT, ATTESTATION).
4. Identify and assess an individual's current needs and goals to create an initial care plan.
5. Complete initial billing questionnaire based on information gathered at intake and enrollment.
6. Complete intake package and intake progress note and upload all documentation into electronic health record within 24 hours of completed intake.
7. Document all screening/referral data daily in applicable spreadsheet.
8. Conduct community-based engagement (including, but not limited to outreach and events) efforts to promote Health Home Program services.
9. Collaborate with clinic-based staff, agency staff, and Community Partnerships.
10. Provide site cross-coverage to ensure that Health Home services are not interrupted.

Minimum Requirements:

• Bachelor’s Degree in a health related field is preferred; GED or equivalent is required, with community based experience.

Excellent interpersonal, public speaking, and written communication skills.

Ability to work autonomously.

Demonstrate professionalism, respect and ability to work in a team environment.
