Job summary

Apscareerportal is seeking an Intake Specialist to join the Housing Services Team. You will screen eligibility, collect documentation, and enter data to support rental and utility assistance programs.

Collaborate with the Program Manager, Coordinator, and Resource Navigator to guide individuals and families through the intake process with trauma-informed care and accurate recordkeeping. The role emphasizes rapport-building, attention to detail, and the ability to communicate with diverse

Qualifications

• Bachelor's degree in social work or related field with at least two years of experience.
• Experience working with vulnerable/at-risk populations.
• Ability to conduct eligibility screenings and document accurately.

Responsibilities

• Orient individuals and families to the community outreach fund and as first contact.
• Determine eligibility and admissions based on guidelines and complete trauma-informed intake.
• Complete Community Fund documentation and assessments according to policy and procedure.
• Track and regularly update community assistance referrals utilizing internal tracking mechanisms.
• Track and regularly update an active tracking mechanism for utility amounts, rent amounts, and other pertinent information.
• Establish and maintain consistent verbal and written communication between all involved parties; liaise between individuals, families, landlords and utility companies.
