Job summary

Hope the Mission is seeking an Intake and Service Specialist to welcome participants at the Navigation Center in North Hollywood. You will conduct intake via HMIS and CES, connect clients to housing, mental health, and hygiene services, and document encounters with care and accuracy.

Reporting to the Program Manager, you will coordinate referrals, maintain records, and ensure HIPAA-compliant handling of sensitive information while delivering trauma-informed, respectful support in a busy,

Qualifications

• High school diploma or GED required; higher degree preferred.
• Minimum 1 year of direct service or front-facing role in social services.
• Familiarity with HMIS, CES, or similar data systems.
• Bilingual English/Spanish preferred.

Responsibilities

• Greet participants with professionalism, kindness, and dignity.
• Serve as primary contact for Navigation Center services in person, by phone, and via email.
• Maintain and administer participant records in HMIS.
• Ensure confidentiality and secure storage of all participant files.
• Coordinate intake and screening; administer CES triage tools as required.
• Facilitate access to the Coordinated Entry System and identify services for participants.
• Schedule intake appointments and coordinate housing interventions.
• Support site safety and coordination of on-site amenities.
• Respond to emergencies and ensure smooth participant movement across services.
• Perform other duties as assigned.
