Job summary

Department of Health and Human Services (DHHS) is seeking an Intake and Eligibility Caseworker to support Aging and Disability Services. The role requires evaluating referrals, meeting with individuals and families, and documenting eligibility information in the Evergreen system.

Travel across Sagadahoc, Lincoln, Knox and Waldo counties is routine. You will provide outreach on intake processes, conduct group presentations, and may handle a small amount of state case management.

Qualifications

• Bachelor's degree in Social Work/Social Welfare; OR Bachelor's degree in a related social service area.

Responsibilities

• Respond to referrals for Developmental Services Case Management eligibility for adults with intellectual disabilities and/or autism.
• Meet with individuals, families, case managers and other stakeholders during intake to assess needs and gather information for eligibility.
• Enter information in Evergreen for intake and eligibility purposes by required deadlines.
• Complete outreach and education on intake and eligibility in school settings.
• Provide state case management for a small number of cases.
• Maintain referral reports and submit timely to supervisor.
• Gather information to document facts for eligibility determinations.
• Meet with transition specialists for case review.
• Notify and consult with Casework Supervisor as needed.
• Assess crisis situations and arrange emergency intervention if required.

Education

Bachelor's Degree in Social Work or related field
