Job summary

Qualifications

• Degree in human services or related field required or equivalent experience.
• Strong communication, attention to detail, and organizational skills.
• Bi-lingual English/Spanish preferred; crisis intervention skills helpful.

Responsibilities

• Intake, screening and triage for Medicaid eligibility using standardized tools.
• Input assessments into the system and track eligibility through EMS/eCIRTS.
• Explain Medicaid rules and assist with documentation for applications.
• Coordinate with DCF/ACCESS staff for eligibility matters and referrals.
• Assist in gathering information and forms, including home visits if needed.
• Educate service providers and ADRC access points to raise awareness of resources.
