Job summary

Delaware Department of Health and Social Services (DHSS) is seeking a Health/Human Service Case Manager I for the Central Intake Unit (CIU). The role involves processing referrals, eliciting confidential information, and determining Medicaid and SNAP eligibility while coordinating with multiple partners.

The incumbent will use automated systems, develop service plans, prepare case histories, and maintain comprehensive records.

Qualifications

• Associate's degree or higher in Behavioral or Social Science or related field.
• Three years' experience in health or human services support.
• Six months' experience in data entry and using automated information systems.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients to determine eligibility and connect clients with health and human services.
• Develops a service plan in collaboration with hospitals, clinics, agencies and other resources.
• Evaluates information from client interviews and case history for review.
• Prepares case histories, reports and maintains logs of case activities.
• Uses automated information systems to enter, update and report health/human service data.
