Job summary

The Department of Health and Social Services (DHSS) seeks a Health/Human Service Case Manager I to process referrals, collect confidential financial and medical information, and route applications to statewide eligibility staff for LTC Medicaid and SNAP.

You will enter data into TAP, prepare case histories, coordinate with hospitals and community resources, and provide guidance to clients and families while communicating across cultures.

Qualifications

• Associate degree or higher in Behavioral or Social Science or related field.
• Three years' experience in health or human services support, such as interviewing clients and assessing needs.
• Six months' experience in recommending service plans and eligibility.
• Six months' experience with automated information systems for data entry and reporting.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients, family members, and others to determine eligibility and connect clients with services.
• Develops service plans with hospitals, clinics, and resources.
• Evaluates information from interviews and other sources and routes for review.
• Coordinates with agencies, hospitals, clinics, and resources to expedite processes.
• Prepares case histories and maintains records and logs.
• Uses automated systems to research, review, evaluate, and report on data.
• Communicates across cultures and diverse populations.
