Job summary

The Oregon Department of Human Services is seeking a Human Services Case Manager to connect aging and people with disabilities with federal, state and local programs or resources. This on-site role focuses on case management, eligibility assessment, and advocacy for clients within the community.

Requirements include a valid driver’s license, willingness to travel, and a commitment to equity and service excellence.

Qualifications

• Valid driver’s license required.
• Bachelor’s degree in Behavioral/Social Science, or related field.
• Bachelor’s degree in any field plus 1 year human services experience.
• Associate degree plus 2 years human services experience.
• Three years of human services related experience.

Responsibilities

• Match needs of Aging and People with Disabilities with available programs or resources.
• Conduct case management and screen applicants for long term community-based care and nursing homes.
• Assess eligibility, re-determine needs, and adjust benefits and services accordingly.
• Advocate for clients and aging and persons with disabilities in the community.
