Job summary

The Oregon Department of Human Services is seeking a Human Services Case Manager to help aging and disabled individuals access federal, state and local resources. You will assess eligibility, screen applicants for long-term community-based care, and advocate for clients in the community.

Qualifications

• Bachelor's degree in Behavioral or Social Science or closely related field.
• Bachelor's degree plus one year of human services experience.
• Associate degree plus two years of human services experience.
• Three years of human services related experience.

Responsibilities

• Match needs with programs and resources via case management.
• Screen applicants for long-term care and eligibility.
• Advocate for clients—aging and persons with disabilities.
• Document all aspects of cases in narration.
