Enhabit Home Health & Hospice is seeking a coordination-focused professional to manage patient referrals and act as a trusted resource for referral sources. You will collaborate with branch and sales teams to ensure smooth coordination and transition to services.

The role requires understanding Medicare guidelines for home health or hospice and strong customer service and basic technology skills, including mobile device use. A high school diploma or equivalent is required.

Qualifications

• Must possess a high school diploma or equivalent.
• Two years of experience in the health care industry and one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Demonstrated knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills, including operation of a mobile device.
