Job summary

Enhabit Home Health & Hospice is seeking an intake/referral coordinator to support patient referrals and act as a key resource for referral sources. You will collaborate with branch and sales teams to ensure smooth transitions to services and apply Medicare guidelines governing home health or hospice.

The ideal candidate has 2+ years in health care and customer service experience, with a preference for home health/pediatrics knowledge and strong technology skills.

Qualifications

• Must have a high school diploma or equivalent.
• At least two years of health care industry experience and one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
