Job summary

Enhabit Home Health & Hospice is seeking a referrals coordinator to support patient referrals and serve as a resource for referral sources. You will collaborate with the branch and sales teams to ensure smooth coordination and transition to services.

The role requires a high school diploma or equivalent, at least two years in health care (one year in home health or pediatrics), strong customer service, Medicare guidelines knowledge, and basic tech skills including a mobile device.

Qualifications

• Must have a high school diploma or equivalent.
• At least two years in health care; one year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic tech skills, including operation of a mobile device.
• Preferred: RN or licensed nurse (state license).
• Experience with intake responsibilities and working with a multi-location company is preferred.
