Job summary

Enhabit Home Health & Hospice is seeking a customer-focused professional to coordinate patient referrals and support transition to services. You will work with the branch and sales teams to ensure smooth coordination and compliance with regulatory guidelines.

The role requires 2+ years in health care with home health or pediatrics experience, strong customer service, and basic tech skills including mobile devices. A valid driver license and dependable transportation are needed for field work.

Qualifications

• High school diploma or equivalent required.
• At least 2 years in health care; 1 year in home health or pediatrics.
• Experience in customer service.
• Knowledge of Medicare guidelines for home health/hospice.
• Basic technology skills and mobile device operation.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
