POSITION TITLE: Intake Coordinator, Los Angeles
LOCATION: Los Angeles, Ca
CLASSIFICATION: Non-Exempt
- Work Shift: Monday - Friday 8:30AM-5:00PM. Must be open to possible schedule change to Sunday-Thursday if need arises
- Fridays and any weekend day worked is work from home
- This position will also assist other locations as needed (Ventura)

Position Overview
The Intake Coordinator manages the referral process, acting as a liaison between hospital social workers and the NHF Recuperative Care staff. They ensure accurate and thorough information gathering from hospitals, foster positive partnerships, and provide excellent customer service with clear communication.
Responsibilities
- Serve as point of contact for social workers on referrals, providing updates on referral status.
- Create and initiate client files, ensure referrals are documented correctly and promptly in the database, and coordinate with the interdisciplinary team to finalize referrals.
- Communicate effectively about referrals and admissions.
- Build trusting relationships with hospital and community partners.
- Deliver excellent customer service, demonstrating compassion and good judgment, especially when working with clients facing challenges like homelessness, unemployment, addiction, health issues, or poverty.
- Support marketing efforts, including conducting trainings, site visits, preparing materials, and updating databases and reports.
- Maintain guest census roster, manage intake/discharge processes, and handle insurance care coordination.
- Contribute to quality improvement initiatives and maintain confidentiality of patient information.
- Collaborate with team members, participate in meetings and trainings, and represent the organization professionally.
- Flexibility to adapt to schedule changes and assist other facilities as needed.
- Perform additional duties to meet organizational needs, always following NHF policies and procedures.
Minimum Requirements
- At least 2 years of experience in administrative support, with 1 year in healthcare or social services.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Strong written and verbal communication skills.
- Excellent multitasking, organizational, and time management skills.
- Experience in medical office or recuperative care settings preferred.
- Familiarity with HIPAA preferred.
- Friendly, service-oriented mindset with empathy.
- Bilingual in a language spoken in Los Angeles (preferred but not required).
Physical Requirements
- Work in an office environment with normal conditions.
- Frequent interruptions, ability to read, use a computer, communicate effectively, and perform physical activities such as standing, walking, climbing, lifting, and grasping.
