Program Details
  • Transitional Housing Program: Provides housing and supportive services to young women in foster care or transitioning out of foster care or juvenile justice systems, aged 18-24.
Role Summary

This position reports to the Director of Supportive Housing and involves performing intake coordination and linkage to stable housing, as well as providing focused services to Supportive Housing aftercare clients. Responsibilities include client needs assessment, advocacy, case management, and collaboration with discharge planning teams.

Schedule & Location
  • Schedule: Monday-Friday, 9:00am-6:00pm, with every other Friday off (9/80 schedule)
  • Location: 155 N. Occidental Blvd, Los Angeles, CA 90026
Responsibilities
  • Conduct client interviews, review documents from relevant agencies, and prepare files for review.
  • Provide social work case management and navigation services.
  • Transport clients as needed.
  • Coordinate with Family Advocates for discharge planning.
  • Maintain intake data and generate reports.
  • Serve as TB Liaison for LAHSA.
Qualifications
  • Bachelor's in Behavioral Science with at least two years of experience working with transition-aged youth, or a Master's in Behavioral Science or Human Services.
  • Bilingual in Spanish/English preferred.
  • Valid California Driver's License and acceptable driving record.
  • Physical ability to perform duties including first aid/CPR and operating a vehicle.
