- Work Shift: Monday - Friday 8:30AM-5:00PM, with occasional Saturday and Sunday
- This position may assist other locations, such as Ventura

Intake Coordinator
The Intake Coordinator manages the referral process, acting as a liaison between hospital social workers and NHF Recuperative Care staff to ensure a smooth transition from hospital to our program. Responsibilities include gathering accurate information from hospitals, fostering positive partnerships, providing excellent customer service, and ensuring effective communication.
Responsibilities and Initiatives
To help NHF meet growth goals, the Intake Coordinator will:
- Serve as the main contact for social workers regarding referrals, providing updates on referral status.
- Create and manage client files, document referrals accurately and promptly in a database, and coordinate with the interdisciplinary team.
- Communicate effectively with team members about referrals and admissions.
- Build trusting relationships with hospital and community partners.
- Deliver excellent customer service with compassion, especially when dealing with clients facing difficult life circumstances.
- Support marketing efforts, including trainings, site visits, preparing materials, and updating databases.
- Maintain guest census roster and manage intake/discharge processes.
- Coordinate insurance care in collaboration with the Social Services department.
- Contribute to programming evaluations for quality improvement.
- Maintain confidentiality of patient information.
- Participate in team meetings and trainings.
- Provide updates during board review meetings.
- Project a professional image and adhere to dress code.
- Adapt to schedule changes as needed.
- Assist the team as required and support other facilities when needed.
- Complete additional duties to meet facility needs.
- Follow NHF policies and procedures.
Requirements
- Minimum of 2 years in administrative support, with at least 1 year in healthcare or social services.
- Proficiency in Microsoft Word, Excel, and Outlook.
- Strong written and verbal communication skills.
- Excellent multitasking, organizational, and time management skills.
- Experience in medical or recuperative care settings (preferred).
- Familiarity with HIPAA (preferred).
- Friendly, service-oriented, empathetic attitude.
- Bilingual in a language commonly spoken in Los Angeles (preferred but not required).
Physical Requirements
- Work in an office environment under normal conditions.

- Read materials and use a computer.
- Communicate effectively with co-workers.
- Stand, walk, climb, stoop, kneel, lift, pull, push, and use fingers for grasping and feeling.
